The professional registered nurse (RN) integrates and practices according to the California Nurse Practice Act in alignment with City of Hope Orange County’s mission, vision, and values. The RN is responsible for implementing the nursing process and collaborates with the interprofessional team while maintaining continuous focus on the patient and family.
At the level of Clinical Nurse IV (CNIV), the RN is a proactive agent who actively participates and leads the integration of research, evidence-based practice (EBP), innovation, and performance improvement to the highest level of quality care. A CNIV engages in and role models lifelong learning, supports professional growth of peers and future RNs, and is dedicated to the advancement of the profession of nursing.
Understands and applies the City of Hope Code of Conduct, Code of Organizational Ethics, and Scope and Standards of Oncology Nursing Practice. CNIVs role model a culture of service excellence with an emphasis on collaboration, collegiality, ethics, diversity awareness and cultural competency, quality of care, and resource utilization. CNIVs assure adherence to all policies, procedures, environmental and infection control guidelines, and actively participate in review and update. CNIVs possess leadership behaviors that support their peers on performance, quality improvement, and Nurse Sensitive Indicators (NSI). CNIVs possess integrity, assure confidentiality, and promote the image of the professional nurses through day-to-day interactions with patients, families, and colleagues. CNIVs achieve results through creative problem solving.
CNIVs demonstrate emotional intelligence as role models for healthy and professional behaviors in alignment with the Department of Nursing professional practice model and the principles of Relationship Based Care (RBC).
CNIVs actively apply RBC principles to deliver and promote a work practice environment in which care of self, care of others and care of community can thrive.
CNIVs role model and actively lead collaboration and teamwork to achieve positive working relationships with all nurses, advanced practice providers, physicians, allied health and support staff members, with attention to self-care and healthy work/life balance.
Education / Patient, family, community, and self
- CNIVs demonstrate expert assessment of patient and family to determine patient’s/family’s ability to comprehend and understand education. CNIVs are experts at facilitating comprehension, minimizing/eliminating barriers and providing patient – centered education.
- CNIVs, in addition to meeting the expectation of employing educational strategies such as teach back, inclusion of designated caregivers and validating education of individual patients, CNIVs also educate on a broader level.
- CNIVs contribute to the education of our communities about health, wellness, prevention, and continuum of care issues relevant to the communities.
- CNIVs are leaders in the review of current literature, attendance at educational programs, workshops, and in-services with dissemination of information back to peers.
- CNIVs are committed to lifelong learning, including on-going pursuit of education, both formal and informal. CNIVs strive to exceed the minimum expected level of education for the professional registered nurse (Bachelor’s degree), pursue professional certification and promote participation in continuing education offerings, both internal and external.
- CNIVs, as all California RNs, are required by California state law to complete continuing professional education to maintain licensure. Additionally, CNIVs routinely and actively engage in intellectual curiosity and enrichment.

- Contributes to EBP projects, Process Improvement, Quality Improvement and/or Shared Governance on unit or organizationally
- Demonstrates, supports, and teaches Evidence Based Practice techniques to Nursing and ancillary staff

- Mentors new and existing RN staff and ancillary support staff
- Role models excellence in staff experience by
- Creating a good working relationship, and positively motivating and influencing peers
- Being visionary and proactive in empowering teamwork

- Encouraging peer to peer responsibility and accountability
- Facilitating communication and providing constructive feedback to peers
- Providing effective management of problems and conflicts identified on the unit
- Applying principles of “Care of Colleagues”
- Role models excellence in patient experience by
- Monitoring and ensuring that patient care delivered on the unit is customer focused
- Monitoring the quality of care being delivered by other nursing staff members
- Applying decision-making theory and scientific method to solve patient care problems
- Providing customer service recovery when applicable
- Acknowledging and assisting in providing unit needs to meet patient care
- Applying principles of “Care of Patient and Family/Community”
- CNIVs are expert at communication, clearly articulating expectations and driving positive outcomes.

- Preceptor of new RNs and nursing students in exemplary professional practice
- Charge Nurse
- Leadership role with Shared Governance
- CNIVs are role models for use of evidence-based practice quality improvement methods. CNIVs actively support exemplary delivery of care for patients and family and resolution of quality concerns for patients and families. Utilizing Lean Methodology, the COH City Blueprint and PDSA concepts, the CNIV continually participates in quality improvement.
- All RNs are expected to:
- Follow established City of Hope and department policies, procedures, objectives, performance improvement, attendance, safety, environmental, and infection control guidelines, including adherence to the workplace Code of Conduct and Compliance Plan. Practices a high level of integrity and honesty in maintaining confidentiality.
- Bachelor’s Degree in Nursing required; Master’s Degree in Nursing acceptable if Bachelor's degree is not in nursing.
- Three years related experience (oncology, charge nurse)
- Current California RN license
- Current National certification, must be maintained in order to retain CN IV status (effective February 22, 2022)
- Current Basic Life Support (BLS) through American Heart Association
- Must have proficient / expert oral and written communication and organizational skills
- Bilingual preferred
- Demonstrates cultural competence
- Maintains professional appearance, per dress code policy
- Demonstrates outward manifestations of City of Hope culture of excellence

- Makes recommendations based on data
- To protect the health of patients and staff and to comply with new State of California mandates, City of Hope staff are required to show proof of full vaccination by September 30, 2021. Compliance is a condition of employment.
